Wauconda Care is five-star rated facility designated by the Centers for Medicare and Medicaid Services. Only the top ten percent of nursing facilities achieve the high rating, which makes this distinction a coveted accomplishment. The rating system was created to assist consumers, their families, and caregivers so that they can easily compare skilled nursing facilities and improve access to information. Ratings are based on scores for health inspections, staffing levels, and quality measures.

This rating system grew out of the Omnibus Reconciliation Act of 1987 and quality improvement campaigns such as the Advancing Excellence in America’s Nursing Homes, which is a coalition of consumers, health care providers and nursing home professionals. Every year, Wauconda Care undergoes a rigorous on-site survey to attain The Joint Commission’s Nursing Care Center accreditation as well as the Post-Acute Care certification. The Joint Commission expert surveyor evaluated Wauconda Care in the areas of advanced rehabilitation services and the needs of all patients and residents, which included infection prevention and control, leadership, and medication management.
